Adverse Event Reporting

The Washington State Academy is tracking adverse events.

All credentialed members of the Academy of Nutrition and Dietetics agree to abide by a Code of Ethics, which includes provisions to ensure protection of clients and the public.  Unfortunately, not everyone who practices in the field of nutrition is concerned about the well-being of the consumer.  In instances that an individual was harmed (physically, emotionally, and/or financially) by a nutrition practitioner, it is important that these situations are reported to the proper authorities to ensure continued public safety and the credibility of the field of nutrition and dietetics. 

If you believe that an adverse event has occurred, the following form is meant to guide you through the process of determining whether it is a reportable offense and to whom this information should be submitted. 

If the individual(s) that you suspect of causing harm is/are a RDN, CDN, CN or other credentialed health professional, you should report that information to the Washington State Department of Health and/or the appropriate licensing board for that profession.
